## Duties and Responsibilities
Coordinate with patients, families, and caregivers to facilitate home health service setup during weekends.
Handle telecommunications efficiently by answering calls and responding to emails pertaining to home health care inquiries.
Manage scheduling of healthcare professionals and adjust rosters based on patient needs and staff availability.
Ensure all intake forms are accurately completed and processed in the organization's patient management systems.
Collaborate with clinical staff to inform them of new intakes and any specific patient needs or requests.
Conduct initial screenings of patient needs and align those with the available resources and healthcare solutions.
Maintain complete confidentiality of all patient information, adhering to HIPAA regulations and company policies.

## Requirements
Proven experience in intake coordination, scheduling, or relevant administrative positions within healthcare.
High school diploma or equivalent required; additional certification in healthcare administration preferred.
